Memorial's Health Information Management department oversees the management of all medical records and release of information for care provided to patients at all locations across the Memorial Health System.

Accessing and managing your medical records is simple and secure with Memorial Health System. Whether you need to request personal records, provide documentation to a third party, or navigate birth and death record inquiries, our Health Information Management (HIM) Department is here to assist you.

We offer multiple options to ensure you receive your records conveniently and efficiently, while adhering to HIPAA and Mississippi statutes for the protection of private health information.

Requesting Medical Records

Online Portal

  • Use our most efficient method by clicking
    Request Medical Records.
  • Upload a copy of your Driver’s License or another government-issued photo ID.
  • Once processed, records will be sent directly to your email.

Email

  • Download and complete the Medical Records Request Form
  • Email the completed and signed form to himroi@mhg.com along with a copy of your photo ID.

In-Person

General Medical Record Requests Answers

You can request medical records using the following methods:

  • Online Portal:
    • Click REQUEST MEDICAL RECORDS.
    • Upload a government-issued photo ID.
    • Records will be securely emailed to you once processed.
  • In-Person or Email:
    • Download and complete the Medical Records Request Form.
    • Submit the form in person at the Health Information Management Department, via email to himroi@mhg.com, or by mail/fax.

Acceptable photo IDs include:

  • Driver’s License
  • State-Issued ID
  • Military or VA ID
  • Passport
    If applicable, provide additional documentation such as Power of Attorney for Healthcare or Legal Guardianship Papers.

Yes. Third parties (e.g., family members, attorneys, insurance companies) must provide proper documentation, such as Power of Attorney or Executor of Estate paperwork.

Fees may apply for printed records or supplies. Details will be shared during the processing of your request.

Answers For Former Merit Health Biloxi Patients

Merit Health Biloxi is now Memorial Hospital Biloxi, part of Memorial Health System.

Access depends on the date of service:

  • On or before November 9, 2024: Use the “My Health Home” portal.
  • On or after November 10, 2024: Access the “My Health Home” portal.
  • Summer 2025: Portions of your records will be available through the myMemorialConnection portal.

We recommend visiting Memorial Hospital Biloxi in person for secure and accurate retrieval.
