The Outpatient Congestive Heart Failure (CHF) Clinic is an area in which patients may receive intravenous medications for treatment of heart failure. Major emphasis is placed on patient education and reducing disease progression. Lab monitoring is also an integral part of services offered. The clinic aims to improve your quality of life and reduce hospital admissions.
Adult Outpatient Infusion Clinic
The Adult Outpatient Infusion Clinic provides important medications and services ordered by your physician. In the outpatient setting, patients may receive infusions such as such as blood and blood products, intravenous medications—antibiotics, hydration, vaccines, and other injections, without having to be admitted to the hospital.
The Coumadin Clinic was established to help manage and monitor patients on the medication warfarin (Coumadin) and provide anticoagulation education to patients. RN Staff at the clinic will draw and monitor lab work as well as adjust Coumadin doses according to your physician’s directions.

Congestive Heart Failure Telemanagement Program (CHF)
The CHF telemanagement program provides patient education and assessment as well as regular follow up telephone calls to patients at home by a clinic RN to promote healthy lifestyles, answer questions, and encourage compliance with doctor’s recommendations. These services help improve quality of life and reduce hospital admissions.
