Comprehensive Primary Care Services from Memorial

Primary Care Providers attend to your general healthcare needs. You will benefit from a long-term relationship with a PCP who knows your medical background and understands what is important to you. Our staff is committed to providing primary care services to patients of all ages. Traditionally, PCPs fall into three specialties: Family Medicine: treating patients of all ages; Internal Medicine: treating adults; Pediatrics: treating children.

Primary Care Services

Once you establish a relationship with a Primary Care Provider (PCP), they will become familiar with your health and can determine what care you may need including:

  • Diagnosis and treatment of illnesses and injuries
  • Preventive Care
  • Counseling
  • Vaccinations and Immunizations
  • Annual Health Checkup / Annual Wellness Visits (AWV)
  • Coordinated care of chronic conditions such as diabetes, blood pressure, breathing disorders and more
  • Referral to a specialist for specific medical needs

Medicare Annual Wellness Visit

If you have Medicare, you may be eligible for a free wellness visit each year. This visit can help you stay healthy and catch serious health issues early.
