Memorial Health System Adopts the Watershed Health Platform to Drive Better Patient Outcomes in the Community
NEW ORLEANS, LA (March 31, 2022) – Watershed Health proudly announces today the adoption and launch of the company’s post-acute and care transition platform by Memorial Health System in Gulfport, MS.
Through the adoption and utilization of Watershed’s software and services, Memorial expects to further establish improved interoperability and connectivity within their acute, post-acute and social determinants of health (SDoH) networks to drive better patient outcomes throughout Mississippi’s Gulf Coast region.
“As we continue celebrating Memorial’s 75 years of service to the Mississippi Gulf Coast, the entirety of our health system remains committed to the goal of keeping our community healthy in every action we take,” said Memorial Health System President and CEO, Kent Nicaud. “That includes utilizing the latest technology available – like the Watershed Health platform – to improve care management for our patients.”
Memorial Health System’s ability to streamline communications between providers within Watershed’s closed-loop system helps doctors and network providers more closely monitor the progression of patients throughout post-acute care and care transitions, ultimately leading to decisions and collaborations that improve patient outcomes.
“We are incredibly excited to launch the Watershed platform with Memorial Health System,” said Watershed Founder and CEO, Chip Grant. “Memorial has a long history of being first to adopt practices that generate medical advancements for the Gulf Coast. We believe this partnership represents another large step forward, connecting provider communities and improving interoperability in service to their patients.”
