Requesting Medical Records
Online Portal
- Use our most efficient method by clicking
Request Medical Records. - Upload a copy of your Driver’s License or another government-issued photo ID.
- Once processed, records will be sent directly to your email.
- Download and complete the Medical Records Request Form
- Email the completed and signed form to himroi@mhg.com along with a copy of your photo ID.
